Bone is one of the few tissues in the body that heals by regenerating itself — but only if the broken ends are adequately aligned and held still. That single principle decides whether your fracture needs a cast or an operation.
When a cast is enough
Undisplaced or minimally displaced fractures in well-aligned positions heal reliably in a cast or brace. Children heal especially well because their bones remodel as they grow.
When surgery is needed
- Displaced fractures that cannot be held aligned in a cast
- Fractures involving a joint surface — even 2 mm of step-off accelerates arthritis
- Fractures of the thigh and certain leg bones, where casting means months in bed
- Open (compound) fractures and fractures with nerve or vessel injury
Malunion and nonunion
A fracture that heals crooked (malunion) or fails to heal (nonunion) causes persistent pain and deformity. These are correctable — with realignment surgery, stable fixation and, where needed, bone grafting. Early specialist care prevents most of them.

How long does a fracture take to heal?
Most adult fractures unite in 6–12 weeks, depending on the bone, blood supply, fixation stability, and factors like smoking and diabetes which slow healing.
Will the plates and screws be removed later?
Usually not — implants are designed to stay safely. Removal is considered only if they cause irritation or for specific reasons after healing is complete.
Can I walk with an implant in my leg?
Yes. Modern internal fixation is designed to allow early movement, and in many cases early weight-bearing, which actually stimulates bone healing.
